Our S.O.S team goes into Homeless encampments, develops positive relationships with the individuals over time and gets them resources and help to get them back on their feet.
Customized & Assisted Living Placements
24-hour Customized Living services for adults living with physical and/or mental disabilities in a comfortable residential setting.
Housing Stabilization Services
A new Minnesota Medical Assistance benefit to help people with disabilities, including mental illness and substance use disorder, and seniors find and keep housing.
Residential ALF Placement Services

We also coordinate tours and facilitate direct connections between individuals, their support teams, and housing managers.
