What is Housing Stabilization Services?
Housing Stabilization Services, is a new Minnesota Medical Assistance benefit that helps Individuals with disabilities, including mental illness, substance use disorder, and seniors find and keep housing.
This service does not require the individual to be on a waiver in order to receive it. The only requirement is that the individual is approved for Medical Assistance.

Housing Transition
Eligibility Criteria
- Enrollment in Medical Assistance (MA): The individual must be currently receiving Medical Assistance to qualify for housing transition services.
- Age Requirement: Applicants must be 18 years of age or older to access these services.
- Documented Disability or Disabling Condition: A documented disability or disabling condition is required for eligibility.
- Experiencing Housing Instability: The individual should be facing challenges related to unstable housing or homelessness.
Services Provided
- Customized Housing Search: We provide a personalized housing search that takes into account each client’s specific needs, preferences, and financial situation to find the most suitable housing options.
- Application Process Assistance: We assist clients throughout the housing application process, including offering support for those with challenging rental histories, to enhance their chances of securing a home.
- Coordination with Housing Providers: We work directly with landlords and housing providers to negotiate terms and secure agreements that are in the best interest of our clients.
- Support for Special Needs: We identify and arrange any necessary modifications or accommodations in new housing to ensure it meets the client’s health requirements and accessibility needs.
Housing Sustaining
Eligibility Criteria
- Enrollment in Medical Assistance (MA): The individual must be a current recipient of Medical Assistance to be eligible for housing sustaining services.
- Age Requirement: Services are available to those who are 18 years old or older.
- Documented Disability or Disabling Condition: A documented disability or disabling condition is necessary for eligibility.
- Need for Housing Stability: The service is aimed at individuals who are seeking to maintain their current housing or enhance their housing stability.
Services Provided
- Financial Planning and Management: We offer guidance and support in budgeting, financial planning, and managing housing-related expenses to help clients sustain their housing over the long term.
- Property Management Assistance: We assist clients with the complexities of property management, including understanding leases, knowing their rights and responsibilities, and navigating issues with landlords or housing providers.
- Access to Community Resources: We connect clients with local community resources that support housing stability, such as utility assistance programs, home repair services, and legal aid.
- Advocacy and Mediation: We act as advocates for clients in negotiations with landlords, housing authorities, and other entities to resolve disputes and address issues that may threaten their housing stability.
